How to get support

Charity fund “Modern Village and City” is an independent organization that offers free help for people who are held in hostage or with serious illness in difficult circumstances.

If you or your child have a serious illness and you need a treatment and you still unable to cope with this problem, our foundation will make every effort to help you or your child get well!

Please provide us with the information as soon as possible.

In case of illness:

1. A detailed letter about the situation:

• Surname, name and patronymic of the patient, name of parents
• Age (full address and contact numbers (home net., Mob), diagnosis,
• Structure and financial situation of the family, parents’ place of work
• Complete medical history, name and phone number of your doctor.
• Вetail information about all expences (surgery, rehabilitation, and so on)
• The amount you need to collect
• Period of the scheduled treatment (for example, if the operation is scheduled for a certain date)

Add to the letter:

2. Photocopy of passport and identification number (passport of parents)
3. Photocopy of birth certificate (passport)
4. Photocopy of certificate of disability (if applicable)
5. A photocopy of certificates, medical statements from the hospital confirming your request
6. A photocopy of a prescription for drugs (consumables) with clearly specified quantity and dosage, treatment or referral to another agency
7. A few photos of child before and during treatment. Photos should be sent by email to our Foundation – [email protected]
8. Statement.

• surname, name and patronymic or full name of the organization
• full address and contact numbers (home net., Mob)
• information about your needs (equipment, machinery, gifts, clothing, and so on.)

2. Photocopy of passport and identification number (passport parents (or guardians) copies of documents of the organization
3. Description of the necessary products (brand, series, size, and so on, depending on required)
